Breathlessness, medically termed dyspnea, is described by the American Thoracic Society as a "subjective experience of breathing discomfort that consists of qualitatively distinct sensations that vary in intensity" [1]. It is an uncomfortable awareness of one's own breathing.
While an elevated breathing rate after strenuous exercise is a physiological response, dyspnea that occurs at rest, with minimal exertion, or limits daily activities generally warrants medical evaluation.
What Causes Breathlessness?
Breathing is a complex process requiring coordination between the central nervous system, the lungs and airways, the chest wall muscles, and the cardiovascular system. Alterations in any of these components can contribute to dyspnea.
Clinically, breathlessness is often considered in terms of its onset: acute (developing over minutes to hours) or chronic (developing gradually and persisting).
Acute Breathlessness
Acute dyspnea can indicate a serious medical condition. Potential causes include:
- Asthma Exacerbation: Narrowing of the airways accompanied by inflammation.
- COPD Exacerbation: Worsening of respiratory symptoms in patients with underlying COPD.
- Pulmonary Embolism: A blood clot affecting the pulmonary circulation.
- Pneumonia: Infection affecting the lung parenchyma.
- Pneumothorax: A collapsed lung caused by air in the pleural space.
- Acute Heart Failure: Fluid accumulation in the lungs due to impaired cardiac function.
Chronic Breathlessness
Chronic dyspnea typically develops gradually and may limit exercise tolerance. Common underlying conditions include [2]:
- Asthma: Characterized by variable airway inflammation and hyperresponsiveness.
- Chronic Obstructive Pulmonary Disease (COPD): Progressive airflow limitation, frequently associated with exposure to noxious particles or gases.
- Interstitial Lung Disease (ILD): A diverse group of disorders that can cause scarring (fibrosis) or inflammation of the lung tissue.
- Heart Failure: Chronic impairment of the heart's pumping ability.

- Positionality: Does it worsen when lying flat (orthopnea) or when standing (platypnea)?

- Associated Symptoms: Is there a chronic cough, wheezing, chest pain, or leg swelling?
- Exposures and History: Smoking history, occupational exposures, or past infections such as Tuberculosis.
Important Investigations
Depending on the clinical presentation, initial testing may include:
- Chest Radiograph (X-ray): To assess for structural abnormalities, fluid, or signs of heart failure.
- Spirometry: To evaluate for obstructive or restrictive physiological patterns.
- Electrocardiogram (ECG): To evaluate cardiac electrical activity.
- Blood Tests: Such as a complete blood count or cardiac biomarkers, if indicated.
If the diagnosis is not clear from initial tests, further evaluation may involve:
- High-Resolution CT (HRCT) of the Chest: For more detailed imaging of the lung parenchyma and airways.
- Echocardiogram: To assess cardiac structure and function.
- Cardiopulmonary Exercise Testing (CPET): To help differentiate whether exercise limitation is primarily respiratory, cardiac, or related to deconditioning.

- Heart Failure: Management typically involves medications to optimize cardiac output and manage fluid status.
- Interstitial Lung Disease: Specific therapies depend on the ILD subtype and may include anti-fibrotics or immunosuppressants.
- Rehabilitation: Pulmonary rehabilitation can be a valuable intervention for many patients with chronic respiratory conditions, helping to improve exercise capacity and manage the symptom of dyspnea.
When Urgent Medical Attention is Required
Certain presentations of breathlessness require prompt emergency medical care, particularly if it:
- Develops rapidly over minutes to hours.
- Is accompanied by severe chest pain, pressure, or tightness.
- Is associated with a bluish discoloration of the lips or fingertips (cyanosis).
- Is accompanied by dizziness, fainting, or altered mental status.
- Occurs with coughing up blood.
- Severely limits the ability to speak in full sentences.
